How We Help

Faith Community Pharmacy provides a wide range of medications – free of charge – for many chronic conditions like hypertension, diabetes, asthma, high blood pressure, and more.

If you or someone you know is struggling to afford medication, please contact your doctor or healthcare provider and ask them to send us a completed referral form along with your prescriptions. Once we receive your information, we will contact you to set up an appointment. (If you do not have a relationship with a healthcare provider, please contact us directly at (859) 426-7837 and we will work through the process with you.

1. Obtain a Referral

Ask your doctor or healthcare provider to send us a completed referral form along with the prescription you need.

2. Make an Appointment

Once we receive the referral from your doctor, we will contact you to set up an appointment.

3. Prepare For Your Visit

Below is the documentation you will need to bring to your appointment with us. ALL documentation is required to determine your eligibility.

We will need proof of ALL of the following information at the time of your appointment.

Photo I.D.Faith Community Pharmacy  serves all residence of Northern Kentucky regardless of nationality or immigration status. ID’s from other countries are accepted.

ResidencyPlease bring a recent piece of mail showing your current physical address. We serve patients who reside in the loosely defined Northern Kentucky area. We are unable to serve patients from other states.

Household IncomeThe following are examples of proof of household income needed for each member of the household

Tax returns
Pay stubs / Unemployment
Social Security Award Letter
Pension Statements
Food Stamp letter if zero/no income
***Regardless of income, Faith Community Pharmacy provides 90-days of medication to anyone seeking assistance. For those earning 300% of the Federal Poverty Level or less, medications are provided on an ongoing basis.

For enrolled patients, medications are automatically refilled every 90-days. If patients are able, we request that prescriptions are picked up from our office located at 601 Washington Ave., Ste. 100, Newport, KY 41071 on or after assigned pickup dates. For patients with significant transportation barriers, medications are delivered directly to their homes.

Faith Community Pharmacy is located in WaterTower Square. Please use the entrance at the corner of 6th St. and Washington Ave. Once you pass through the double doors, we are the first office on your right.

Faith Community Pharmacy provides a wide variety of medications for chronic illnesses like heart disease, asthma, and diabetes. You can view the current list of medications we provide. If you don’t see your medication listed please contact us.

We are prohibited by law from dispensing controlled substances. We do not stock and cannot dispense any controlled substances, opioids, contraceptives, or narcotics.

Food as Medicine

Food as Medicine

In 2021, Faith Community Pharmacy conducted research that showed many of our diabetic patients wanted to better manage their disease through diet and exercise, but faced two unique challenges: they weren’t sure exactly what they should be eating and they feared they wouldn’t be able to afford the food even if they did know. This insight inspired us to create partnerships with Be Concerned Food Pantry and the UK Extension Office to provide free, disease specific food education and free disease specific food for our patients and others in our community in need.

How it Works

  • (7) 1.5 hour  training classes are held in Erlanger, KY
    • Patients are provided a $25 Kroger gift card for every class they attend
    • Transportation support if needed
    • Free disease specific food provided at each meeting
    • Access to a registered dietician throughout the course
  • Ongoing disease specific food support post-training

Eligibility

  • Resident of Kentucky
  • Diagnosed as diabetic or pre-diabetic
  • Unable to afford education classes and/or disease specific food
  • Income at or below 300% of the Federal Poverty Level
