Make a Referral

To refer a patient to the pharmacy, please download, complete and fax the referral form to (859) 426-5708 and E-Prescribe (e-Rx) all prescriptions to Faith Community Pharmacy. Once documentation is received, we will reach out directly to your patient.

Patient Eligibility

Patients must be a resident of Northern Kentucky. All patients, regardless of income, are eligible for 90-days of medications. Patients earning 300% of the Federal Poverty Level or less are eligible to enroll for ongoing care.

Referrals will be honored for 90 days upon receipt

Our Formulary

We do not stock controlled substances, contraceptives, or narcotics. As much of our stock is supplied through donations, this list is not all-inclusive or exhaustive, and supply fluctuates on all items marked as Limited Supply. Please call with any questions about this formulary or if you wish to inquire about other medications we may have in small supply.

Ways to Help

Donate Supplies and Medications

We are always in need of supplies and medications to ensure the safety of our staff and help us fill and deliver prescriptions. If you have excess samples or supplies specific to the chronic diseases we treat, please call the office to schedule a pickup.

Other useful supplies include: plastic baggies, garbage bags, paper grocery bags, boxes, staplers and staples, tape, markers, pens, hand sanitizer, cleaning wipes, disposable surgical-grade masks and latex and non-latex gloves.
