Faith Community Clinic is a free clinic providing medical care at no cost for uninsured patients. If you or someone you know does not have health insurance and is struggling to afford care, please contact us at (859) 426-7837. Our team will guide you through the process and help schedule an appointment.-
We will need proof of ALL of the following information at the time of your appointment.
Photo I.D.
Faith Community Clinic serves all residence of Northern Kentucky regardless of nationality or immigration status. ID’s from other countries are accepted.Residency
Please bring a recent piece of mail showing your current physical address.Household Income
Household income is at or below 300% of the Federal Poverty Level. The following are examples of proof of household income needed for each member of the household.-Tax returns
-Pay stubs / Unemployment
-Social Security Award Letter
-Pension Statements
-Food Stamp letter if zero/no income
