Residency/Photo I.D.To receive care, you must be a Kentucky resident regardless of nationality or immigration status. ID’s from other countries are accepted. Please bring a recent piece of mail showing your current physical address and your photo identification.
Household IncomeA patient's annual household income must be at or below 300% of the federal poverty level. Proof of income must be provided. Examples include: tax return, pay stub/unemployment, social security award letter, pension statement or food stamp letter if zero/no income.
InsuraceFaith Community Clinic ONLY serves patients without health insurance.
